Academic Year 2025-2026 Scholarship Application

Education and Verification of Enrollment

1. All applicants shall obtain a copy of your high school transcript and include it with application submittal.

2. If currently enrolled at a college, university, or trade program:

  1. Provide copy of most recent transcript from the institution (if applicable)
  2. Provide proof of enrollment for 2025-2026 academic year (e.g. tuition payment confirmation, enrollment letter from institution).

 

Activities and Accomplishments

On Page 3, applicants are asked to use the table provided to list any clubs or activities with which they had any involvement in during high school or college, provide any accomplishments achieved, and also identify any employment obtained while attending school.

 

Getting to Know the Applicant

In your own words, briefly describe why you have chosen your selected degree/trade to pursue and what goals you have to contribute to said field. You may also use this section to provide additional information regarding Activities not covered on Page 3 if needed.
